You know, I have some guidelines that my manager sent me about what information to include in the performance review.

what to include infromation 인데 information이 what 뒤쪽으로 온건가요.? 아니면 원래 what intormation 인데 형용사적 투부정사가 붙은건가요.?

=>what information to include : 의문사+to부정사: 간접의문문 변형입니다. what(의문형용사)+information(명사)

=what information I should include를 의문사 to부정사로 변형시킨 것입니다
